ACORD 25

The standard industry form used to issue a Certificate of Insurance.

ACORD 25 is the Certificate of Liability Insurance form produced by the ACORD Corporation, the insurance industry's standards body. Nearly every US insurance agency uses this template to issue proof-of-insurance certificates to third parties such as general contractors, property owners, and landlords. The form captures the insured's name, the carriers providing coverage, policy numbers, effective and expiration dates, coverage limits, and space for endorsements in the Description of Operations box. When a GC receives a certificate from a subcontractor, it's almost always an ACORD 25. Reading it correctly is one of the most important compliance skills in construction.
